Lithium is used as a treatment option for mood stabilizer in psychiatric or mental health conditions like acute manic episode, bipolar disorder and recurrent depression. Sid effects of Lithium can vary from gastrointestinal symptoms to central nervous system symptoms like : nausea, vomiting, fine tremors, polyuria, polydypsia, weight gain, leucocytosis, hyperparathyroidism and hypercalcemia. The toxic adverse effects start to occur when the levels of lithium in blood exceeds the therapeutic dose which is less than 1.5 mmol/L. Mild Lithium toxicity occurs when serum Lithium levels are more than 1.5 mmol/liters and the patients present with vomiting, diarrhea, coarseness, tremors, blurred vision, drowsiness, muscle weakness, ataxia and motor incordination. Severe toxicity occurs when the levels are more than 2 mmol/L and the patients will complain of hyper reflexia, hyper extension of limbs, convulsions, renal failure, cardiac failure, coma & death. Lithium toxicity can be precipitated by drug interactions, depletion of sodium caused by vomiting & diarrhea and drug overdosing. So before commencing a patient on lithium, it’s important to carry out a detailed assessment which includes: FBC, S creatinine, S electrolytes, ECG, thyroid function tests and pregnancy tests. Lithium is started only if the above tests are normal.
